Celestial Sol Sessions for minors require advance approval and consent from a parent or legal guardian.Personal information relating to a minor may be collected when reasonably necessary to schedule, evaluate, or provide an approved service and with appropriate parent or legal-guardian involvement.
NEVADA CONSUMER HEALTH DATA PRIVACY NOTICE
Celestial Sol may collect limited consumer health data when reasonably necessary to evaluate safety, suitability, accommodations, or participation in a requested Celestial Sol Session.
INFORMATION WE MAY COLLECT
Depending on the circumstances and information voluntarily provided by a client, this may include:
• Recent surgery, injury, concussion, fracture, or other significant physical event
• Pregnancy or recent postpartum status
• Seizure-related information
• Current conditions or symptoms affecting breathing, consciousness, movement, balance, or safe participation
• Severe or unmanaged mental-health symptoms or a recent acute psychological crisis
• Mobility, sensory, communication, or other accommodation needs
• Other health-related information a client voluntarily discloses because it may affect safe or appropriate participationCelestial Sol does not routinely request a complete medical history, complete mental-health history, medication or supplement list, or trauma history.
SOURCES OF CONSUMER HEALTH DATA
Consumer health data is obtained directly from clients or prospective clients through:
• Safety & Suitability screening
• Intake or consent forms
• Email or direct communication
• Information voluntarily disclosed before, during, or after a sessionCelestial Sol does not purchase consumer health data from data brokers.
HOW CONSUMER HEALTH DATA IS USED
Consumer health data may be used as reasonably necessary to:
• Determine whether a Celestial Sol Session is appropriate at a particular time
• Determine whether a session should be modified, postponed, provided virtually, or require additional screening or healthcare clearance
• Provide reasonable accommodations
• Support safe and appropriate participation
• Maintain records relating to safety and consent
• Comply with applicable law
Celestial Sol does not use consumer health data to diagnose or treat medical or mental-health conditions.
PROCESSING AND SHARING
Celestial Sol does not sell consumer health data and does not share consumer health data for independent advertising or marketing purposes.Consumer health data may be processed by service providers acting on behalf of Celestial Sol when reasonably necessary to operate scheduling, intake, technology, communication, or recordkeeping systems.Consumer health data may also be disclosed when required or authorized by law or when the client has provided any consent required by law.
CLIENT FEEDBACK, TESTIMONIALS AND MEDIA
Consent to receive a Celestial Sol Session does not constitute consent to publicly share consumer health data.Identifying health-related information contained in testimonials, client messages, photographs, video, audio, or other media will not be publicly shared without the client’s specific permission.Ordinary anonymous feedback that does not contain identifying or sensitive health information may be shared for educational or promotional purposes.
NEVADA CONSUMER RIGHTS
Subject to applicable law, Nevada consumers may request that Celestial Sol:
• Confirm whether consumer health data relating to them is being collected, shared, or sold
• Provide information regarding third parties with whom their consumer health data has been shared or sold
• Stop collecting, sharing, or selling their consumer health data
• Delete consumer health data relating to them
Nevada law provides consumers rights concerning consumer health data and requires covered entities to provide a means for exercising those rights.

Celestial Sol may request reasonable information necessary to authenticate the identity of the person making the request. Requests will be handled within the time required by applicable law.
APPEALS
If Celestial Sol declines to act on a Nevada consumer-health-data request, you may appeal that decision by emailing carissa@celestialsol.com with the subject line: Consumer Health Data Appeal
CHANGES TO CONSUMER HEALTH DATA PRACTICES
Celestial Sol may update its consumer-health-data practices from time to time. If a material change requires additional notice or consent under applicable law, Celestial Sol will provide that notice or obtain additional consent before using previously collected consumer health data for the new purpose.
